Drug blunders and other errors
THE article 'Drug blunders hit five patients a day' (May 11) is misleading. The sub-heading implies the article is about prescription errors. In fact, the article refers to prescribing errors, drug errors (which include prescribing, administration and recording errors) and "paperwork and delays".
Also, in spite of the article focusing on blunders over drug prescriptions, by far the majority of which will have been made by doctors, the photograph shows a nurse administering medicines from a ward trolley.
