SCOTTISH women will be advised to ditch the contraceptive pill in favour of longer-lasting and more reliable implants and injections, under a revolutionary change to family planning aimed at reducing the rate of unwanted pregnancies.
Around a quarter-of-a-million Scottish women aged 18-40 who currently use the pill will be told they should consider switching to "long-acting reversible" methods that last from three months to five years.

Only a tiny minority of Scottish women of child-bearing age currently use implants and injections. Health minister Shona Robison will launch the strategy next spring in a bid to cut the growing number of unwanted pregnancies which result in more than 1,000 abortions a month.

Ministers will spend about £270,000 a year publicising the new strategy which was welcomed last night by many doctors and sexual health specialists. But the campaign has also sparked controversy with experts warning it could exacerbate fertility problems among older women wanting to start families because normal fertility can take five or more months to return after some long-term contraceptives stop working.

The strategy is being launched as Scotland's abortion rate stands at a record high. There were 13,703 terminations carried out last year – an average of 38 per day. Forgetting to take a pill or using a condom incorrectly are common causes of unplanned pregnancies.

Just 4% of Scottish women aged 15-49 currently take long-acting contraceptives compared with around 23% who take the pill. A further 18% use condoms and this group will also be targeted in the campaign alongside those who use the patch, caps or diaphragms and women requesting emergency contraception.

The remainder are not using contraception, either because they or their partner have been sterilised, are not in relationships or are trying to conceive.

Last night a Scottish Government spokeswoman said: "The campaign will feature all types of long-lasting, reversible contraception – injections, coil and implants. The aim will be to reduce unintended pregnancies, because of the low failure rate and high reliability rates of these methods."

Robison added: "Knowledge is power and by increasing knowledge and providing better information people will be able to make more informed choices."

As well as advertising the options to women, it is expected that GPs and nurses will be urged to inform patients about long-acting contraceptives during family planning consultations.

The contraceptives concerned include Depo Provera, a hormone injection which offers three months of protection; Implanon, a small rod implanted into the arm which gives three years of protection; the coil; and Mirena, an intra-uterine device which lasts five years.

All cost less than £27 a year per patient compared with £29-£36 a year for some common contraceptive pills.

The pill generated huge social change during the 1960s by giving women control over their fertility for the first time.

However, it must be taken at the same time every day to ensure its effectiveness, making it prone to 'user-failure'. The failure rate of the pill at around 8% is much higher than the 1%-3% for long-acting alternatives.

Lorraine Mann, a sexual health promotion specialist at NHS Highland, said: "The advantage of long-acting methods is that once you have it you don't have to remember to take it every day or every time you have sex, and you will get called back to the surgery when you need to have it again. I very much welcome this development."

One Glasgow-based doctor said: "These long-acting methods are also seen as being reliable and cost effective. Most of these methods are suitable for women with medical conditions such as obesity, high blood pressure, migraines and previous deep-vein thrombosis who can't use the pill. We are seeing more and more fat people, it's becoming a significant problem."

But Dr Stuart Scott, deputy chairman of the British Medical Association's Scottish GP Committee, urged caution. He said: "There are political imperatives to prevent unwanted pregnancies so there are drivers from the Government to change that. But we need to be sure we are not forcing people to have a form of contraceptive they are not happy with, and that's certainly a risk."

One patient, a 35-year-old mother of two from Elgin, Moray, who asked not to be named, said she was deeply unhappy with her experience of taking Depo Provera and blames the drug for the fact she has failed to conceive a third child despite two years of trying.

She said: "I developed quite bad depression, and I put on over two stone even though I hadn't changed my eating or exercise habits. I have also been left with fertility difficulties. I was not warned of any of this when I was given the injection."

A spokesman for Pfizer, which makes Depo Provera, said there was no evidence it caused permanent infertility. He added: "Depo Provera is an option for patients, but has side effects that should be discussed with the patient by the doctor."

Bayer Schering Pharma, which manufactures Mirena, would not comment directly, but referred to its product information leaflet. Schering-Plough, which makes Implanon, did not return calls.

I had one Depo shot in 1999-it is almost 2008-I have had severe depression every single month for nearly 10 years because of Depo. It caused my endometriosis in which I had to have a 7 hour operation to correct and I bled for a year and a half right after the shot. I have never been the same. My doc-if you can call him that-said it would help my cramps. I have a strong family history of depression. Don't take birth control!!!!! It is poison that the drug companies push. Try herbs and diet to regulate your system and condoms for birth control. Depo destroyed my life for almost a decade. I too lost patches of hair and am still trying to get one area to grow more. I also developed cystic acne that is finally gone-but the scars aren't. I wish I could take my old doctor to court.

I need help, It's been at least a year since I have been off of DV I want to be normal again, this shot has done nothing but harm me, the mood swings were awful, the weight gain was fifty pounds, and the facial hair came out of nowhere, I feel like I have a beard and I am only 23 years old! my periods are so random, I cannot keep track of them, Me and my Fiance have been together for six years and I am unable yet to have children I am only 23! I don't feel like a woman anymore, I wan't to cuss this shot up and down so bad but I cannot, I am so upset, so upset that I made a choice to be with a man for the rest of my life and not bring a baby into this world, that we cannot afford to give a good life in this period of time, this is how these ***holes repay us, with no real research on the shot, just throwing it out there and expecting nothing bad to come of it, these people that research this stuff went to school for a long time and worked really hard to get where they are at, but when they let something like this out for the public, it makes me so angry, there is so many things that are wrong with the testing of drugs and etc, it is sad, maybe I should't be so stupid and just go without it all and let nature take its course, I don't take any pills or anything like that, because they all have side effects too.

I believe the man should also take responsibility for family planning be it donning condoms, coitus interruptus, or reversible vasectomies.

Why is it always the women that have to be experimented upon and sometimes with dire consequences as Charles has pointed out in his usual, UNDERSTATED! way.

Currently, there's only one form of contraception that a man can use, and it's perfectly evident to his sex partner whether he's using it. The form concerned impairs the experience for both partners, which is why most people prefer something else once they're past the early stages of a relationship

Re #14 : a vasectomy can be appropriate for a man at a certain stage in his life, but not every man is yet at that stage. Personally I dislike the term "the snip": the aim seems to be to trivialise an invasive surgical procedure which, like any other, can have complications.

I'll tell you why, women contrary to popular belief as not as green as some guys would like us to be.
Women have been used as medical experiments since time and memorial, pills to cure this and potions to cure all..operations which once were considered by the best consultants as the only options!!
If we listened to / or took the advice of every crank who came along, we would all be in the nut house, no doubt being experimented on.

Who is forcing these women to take these drugs, have these procedures, etc?
They are going in voluntarily to their physician for treatment of a condition or situation that they deem intolerable enough to seek treatment.
If medical intervention seems too risky, then let them choose between abstinence and pregnancies. That is the real "natural" way.
But they want to sh*g their brains out, have a decent standard of living and convenient lifestyle, soooo abortions are the answer. But those are medical/surgical procedures with risks and side effects too.
And as for male contraceptive - when men can get pregnant it will become more practical.

Hi boys. I'm a woman and I'm actually quite grateful for the news released today. Here's why...

The "pill" (the combined pill) is not appropriate for all women, and can be dangerous for some. More awareness of other methods is a good thing for everyone!

Safer, more reliable alternatives have been there for over a decade, yet have not been promoted, nor have GPs been actively encouraged to use them.

I've used the pill, Depo Provera and Implanon. The only reason I took the pill was because my GP was negligent. After 18 months of taking it daily, it was revealed that I should never have taken the pill, because of serious medial complications.

So I ended up on Depo (the 3 monthly injection.) I hated it, for personal reasons, and switched to Implanon (the 3 yearly implant.)

I love it.

I only need to think about contraception once every three years! Both me and my partner can breathe easy and not fret because it doesn't rely on me taking a dose every day. We're happy and relaxed, all the time.

The only drawback is if you are obese - the implant is less effective. My advice is to loose weight, get the implant and relax. Life is worth living.

Whatever happend to abstinence? Will this "Depo" prevent STD's?

For those in a long term relationship wh simply get sick of using condoms or for medical reasons don't want to use the pill..it might be an effective solution, however how many "umwanted" pregnancy's are a result of those in stable relationship? ( I really have no idea, very curious though).

Imagine if our teenagers switched to a 3 month injection / implant - no babies being terminated...but probably a hell of a lot more STD's and serious health problems.

Some things work for some people, but not all people. The more choice women have the better as far as I'm concerned.

Eh, I tried to get an implant in Fife and was told they no longer gave them out in this area as so many had to be removed cos of the side effects.

At least with the pill if it doesnt agree with you then you can just stop taking it unlike the implant or jag.
